To All Wedge Wood Owners,
To meet state requirements, we need a signed and dated Revitalization Form from every homeowner. To ensure our records are accurate, we kindly ask that you write your unit number directly underneath the date on the form.
How to Submit Your Form
· By Mail: Use the form you recently received in your mail. 2635 Bilgore Grove Blvd. Palm Harbor, FL. 34684
· In Person: Pick up a new form at the office if you prefer between 10 am and 4 pm, sign it, and leave it there.
· Drop the form off at the office or deposit it in one of our drop boxes.
· In the coming days, Board Members will be walking around the community to pick up the form.

Why This Matters Now
Our association has already invested thousands of dollars to meet these state requirements. We have a firm deadline of May 1, 2026, to submit our package. If we do not have the required participation by then, the current process will lapse. Starting over would cost the community additional thousands in new fees and legal costs.
By taking just two minutes to sign and return your form today, you help us protect our community’s budget and ensure we remain in compliance with state law.
